B. Planning Services

Phase 1: Basic Preparedness

Includes intake discussion, information gathering, and family planning support.
  • $125/hour (up to 3 one-hour visits weekly)
  • $325 weekly flat rate
  • $1,250 monthly flat rate

Phase 2: End-of-Life Planning

Includes basic preparedness (if not already completed), respite support, vigil planning, family education, and grief support.
  • $200/hour (up to 3 one-hour visits weekly)
  • $550 weekly flat rate
  • $1,750 monthly flat rate (includes up to 12 hours of planning & education)

Presence, comfort, and supportive daytime care.
